State Fair Hearing

If you do not agree with our appeal decision, you have the right to request a State Fair Hearing. A State Fair Hearing is an appeal that goes to the Nebraska Department of Health and Human Services (DHHS) instead of Molina. For a State Fair Hearing, Nebraska DHHS will make the final decision.

You have the right to request a State Fair Hearing within 120 days from the day you receive our appeal decision.

You must submit your State Fair Hearing in writing to:

Be sure to include the following:

  • Member’s first and last name.
  • Molina Healthcare ID number. This is on the front of the Member ID Card.
  • Member’s address and telephone number.
  • Explain the problem.

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
