2026 Preventive Health Guidelines

Preventive health guidelines can be beneficial for both providers and patients. These guidelines are based on scientific evidence, reviews of medical literature, or established authority. All recommendations are grounded in published consensus guidelines and do not prioritize treatments based solely on cost considerations.

These guidelines aim to recommend a standard level of care but do not preclude the delivery of additional preventive services based on the individual needs of each patient.

Senior Whole Health of New York (SWH of NY) regularly reviews and updates these guidelines as new clinical evidence emerges. After review and necessary modifications by the Quality Improvement and Health Outcomes Committee (QI-HOC), providers are notified of any changes via fax and/or mail.

The guidelines listed below were reviewed and adopted by the Quality Improvement and Health Outcomes Committee (QI-HOC), with approval dates indicated for 2024 and beyond. All guidelines undergo quarterly reviews and receive annual approval unless otherwise noted.

It's important to note that clinical guidelines are not used by SWH of NY to determine benefit coverage. Instead, they are educational tools to aid clinical decision-making and are not considered a standard of care. Physicians should adapt the guidelines based on their clinical judgment.
