Molina One Care (HMO D-SNP)
Molina One Care (HMO D-SNP) is a Medicare Advantage Special Needs plan for age 21 and 64 years old that combines your Medicare, MassHealth Standard (Medicaid) and Medicare Part D benefits in one convenient plan.
You are eligible to join the Molina One Care (HMO D-SNP) plan if:
o You reside in the geographic service area
o – and – you must have both Medicare Part A and Medicare Part B
o – and – you are eligible for MassHealth Standard (Medicaid) or MassHealth CommonHealth
