How To Enroll

You can enroll in Molina Dual Options STAR+PLUS Medicare-Medicaid Plan:

  1. Call the STAR+PLUS helpline at (877) 782-6440, TTY: 711 or (800) 735-2989, Monday to Friday 8:00 a.m. - 6:00 p.m. Tell them you want to enroll in Molina Dual Options STAR+PLUS MMP.

  2. If you would like a personal appointment with a licensed Molina Healthcare agent to learn more, please call (855) 895-9992 TTY 711: Monday to Friday, 8:00 a.m. – 8:00 p.m. local time.
