Pregnancy Resources
Submit the appropriate form to help pregnant members keep their Medicaid eligibility and connect them to services and supports:
- Report of Pregnancy (ROP) – Submit upon identification of pregnancy.
- PRAF 2.0 – Ohio Medicaid Web-Based Perinatal Risk Assessment Form – Submit at the first prenatal visit, at the start of postpartum period, and when there are changes in the member’s medical condition, social risk factors or needs.
