Confidential Communication Requests - Medi-Cal Members Only

A “Confidential Communication Request” ("Request") is a request by a Member that Plan communications be sent to them in private. This applies when they include Medical information ("MI"). The MI will be sent to your chosen address, email, or phone number. We will accommodate your request if the MI can be sent:

  • in the requested form and format, or
  • to other places

 

Requests must be in writing. You may use our form. Please click here. You can also call us. Please use the phone number on your Molina ID card.

Note: A Confidential Communication Request form is not used to update demographic information on your account. If you would like to update your address, email, or phone number, please contact Member Services at the toll-free number on the back of your Molina ID card.

 

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
