Your Child’s Plan Benefits
Children’s Medical Services (CMS) Plan by Molina Healthcare, starting October 1, 2026, will provide all Medicaid covered services to support your child’s health and well being. These services will include, but will not be limited to:
- Doctor and office visits
- Behavioral health services
- Hospital and outpatient services
- Home health care
- Pharmacy services
- Other Medicaid‑covered medical services
Some services may be covered directly by Medicaid rather than CMS Plan. If you have questions about coverage, you will be able to contact CMS Plan at (800) 262-0750 (TTY: 711) for more information.
Expanded Benefits
Expanded benefits will be extra goods or services provided to you at no additional cost. These benefits will go beyond standard Medicaid coverage.

Transportation Services
If your child will need a ride to any covered medical service, transportation will be available at no cost to you.
- You will call MTM at (888) 298‑4781 (TTY: 711) to schedule a ride.
Changes to Benefits
If there will be any changes to covered services or other benefits that affect your child, we will notify you in writing at least 30 days before the change takes effect.

Behavioral Health In Lieu of Services
Behavioral health services will be optional that may be chosen instead of more traditional services, based on your child’s individual needs.

Children’s Medical Services (CMS) Plan by Molina Healthcare of Florida will provide managed care services to our members. Molina Healthcare of Florida is a licensed Florida health plan. CMS Plan will provide free aids and services to people with disabilities, such as qualified sign language interpreters, written information in other formats (large print, audio, accessible electronic and other formats), and free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages. This information will be available for free in other languages. Please contact Member Services at (800) 262-0750 (TTY: 711) Monday - Friday, 8:00 a.m. to 7:00 p.m. local time.
