Welcome to Children's Medical Services Plan

CMS

Children’s Medical Services (CMS) Plan by Molina Healthcare will provide managed care services to children throughout the state. Molina Healthcare is a licensed Florida health plan.

Eligibility

CMS Plan is for children who:

  • Are under age 21 and eligible for Medicaid
  • Have special healthcare needs that require extensive preventive and ongoing care

There are two different versions of the CMS Plan.

  • Members who will qualify for the CMS Plan through Florida Medicaid will be enrolled in the Title 19 plan. If you do not pay a monthly premium for your child’s health insurance, then your child will be enrolled in this plan and the information on this page applies.
  • Members who will qualify for the CMS Plan through Florida KidCare will be enrolled in the Title 21 plan. If you pay a monthly premium for your child’s health insurance, then your child will be enrolled in this plan and not all the information on this page will apply. Getting Started

Every CMS Plan member will have a dedicated Care Manager who will help with their care. It’s important to know your child's Care Manager so your family will get the support it needs. Your child’s Care Manager will contact you to set up a time to come to your home to meet you and your child.

Care Management

Your child’s Care Manager will guide you through the healthcare system and work with your child’s providers to identify their needs.

Care Managers will also:

  • Schedule doctor visits, medical tests and procedures
  • Help you find specialists
  • Suggest behavioral and developmental services
  • Explain benefits and services
  • Connect your family to support services

Children’s Medical Services (CMS) Plan will partner with Molina Healthcare of Florida to provide managed care services to our members. Molina Healthcare of Florida is a licensed Florida health plan. CMS Plan will provide free aids and services to people with disabilities, such as qualified sign language interpreters, written information in other formats (large print, audio, accessible electronic and other formats), and free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages.
